Complete the Pledge Form
Download and complete the pledge form and return it to neuromod@umn.edu no later than April 14th, 2025. If you are working with an individual at UMN, please ensure you always copy neuromod@umn.edu when submitting your pledge form. If you are not a current University of Minnesota supplier or have never sponsored before, please also submit the company’s W9 form so we can set you up as a supplier.
Watch for Your Invoice
Once your sponsorship is processed, you will receive an invoice via email. Please monitor your inbox, including junk or spam folders, for emails from billing@umn.edu or yang8421@umn.edu.

Note: Payment is due within 30 days of receiving the invoice.
